Walking Each Other Home is an integrative palliative care and end-of-life navigation practice founded by Dr. Julie Johns, a board-certified emergency medicine physician with over 20 years of experience in acute care and a passion for conscious living and dying.

At Walking Each Other Home we know how difficult it can be to navigate the complexities of our healthcare system. We are here to help you better understand your illness, communicate your healthcare goals, and what living, and dying, looks like for you.

Palliative Care Coaching with Dr. Johns may include meeting with you and your family to assure alignment with, and understanding of, your unique situation and goals. Together we can create a communication plan for the patient, family, or caregivers for doctor appointments. This may even involve Dr. Johns accompanying the patient and family to future appointments, in person or virtually, in order to translate complex medical information through the lens of your personal goals of care.

Having your goals and wishes clearly stated is important for the final chapter of your life. If need be we also can help navigate the hard conversations with your family, with the utmost respect for autonomy and dignity.
